Put the two halves together above both your testicles and carefully close them together and make sure no skin is caught between them before you close the single screw. Black Label preferred this bit old-skool screw-closure method over a magnetic closure solution, because it offers more control, it avoids pinching of the skin and it will not drop off when you don’t want it to.
 
The inner diameter of this ball stretcher is 35 mm, which is a perfect medium size, suitable for most customers regardless of how well hung they are.
